Job summary

Summa Home Health and Hospice LLC is seeking an LPN to provide skilled nursing care in patients’ homes under physician orders. You will educate patients and families, monitor conditions, and ensure documentation adheres to agency and Medicare guidelines.

The role requires LPN licensure, strong communication, and compassionate patient care. On-call duties and coordination with the RNCM are part of the responsibilities, with travel to patient homes as needed.

Qualifications

• One year of LPN experience in an acute care setting is strongly recommended.
• Hospice care experience is desired.
• Ability to communicate with patients, families, physicians and co-workers.
• Detail-oriented and observant.
• Valid driver’s license and auto insurance in your name.

Responsibilities

• Provides skilled nursing care in a patient’s home as ordered by the attending physician/medical director.
• Performs care according to order sets and goals by RN.
• Observes and monitors patient conditions; notifies RNCM of changes.
• Reconciles medications prescribed by the physician.
• Counsels patient and family on disease progression and management.
• Completes clinical documentation per agency protocol and Medicare guidelines.
• Participates in on-call requirements and case conferences.
