Job summary

St. Camillus is seeking a compassionate Hospice LPN to provide skilled nursing support to hospice patients under the direction of an RN Case Manager or Hospice Manager.

The role focuses on individualized, comfort-focused care and maintaining patients' physical needs, emotional well-being, and dignity. Responsibilities include delivering nursing care under supervision, documenting visits and vital signs, and communicating changes in condition.

Qualifications

• Licensed as a Wisconsin LPN with current license.
• At least one year of medical-surgical experience as an LPN.
• Hospice or home care experience preferred.
• Current CPR/BLS certification.
• Strong communication, documentation, and customer service skills.
• Ability to safely perform essential job functions with or without accommodation.
• Valid driver’s license and driving record compliant with policy.

Responsibilities

• Provide nursing care to hospice patients under RN supervision and plan of care.
• Document vital signs, observations, visits, and communications promptly.
• Communicate changes in condition to the RN Case Manager or Hospice Manager.
• Support comfort, safety, dignity, and quality of life for patients and families.
• Follow St. Camillus policies, safety procedures, and confidentiality standards.
• Participate in care coordination, team communication, and mandatory meetings.
• Provide reliable, flexible on-site and on-call support as required.
