Mercy Health Home Care and Hospice by Compassus is seeking a Hospice Licensed Practical Nurse (LPN/LVN) to deliver compassionate, skilled nursing care in home settings. The role reports to the RN Case Manager and focuses on comfort, dignity, and quality of life for patients and their families.

Responsibilities include implementing the Plan of Care, educating caregivers, and collaborating with the care team to ensure safe, effective care delivery.

Qualifications

• Active nursing license in the applicable state (LPN/LVN).
• Hospice or home health experience is beneficial but not required.
