Job summary

Tides Hospice LLC is seeking a compassionate Home Health Aide (HHA) to support terminally ill patients in home-based and Assisted Living settings. You will work under RN supervision as part of the Hospice interdisciplinary team to promote comfort, dignity, and quality of life.

Responsibilities include personal care, mobility assistance, meal prep, and reporting changes to the RN. Must travel between assigned locations with a valid driver's license and maintain required certifications.

Responsibilities

• Provide personal care including bathing, oral care, skin care, and hair care.
• Assist with toileting, bedpans, and commodes.
• Assist with bed transfers and ambulation per care plan.
• Perform range-of-motion exercises as instructed by qualified personnel.
• Prepare meals, including meals for special diets.
• Perform essential incidental household tasks that support the patient's comfort and well-being.
• Assist with shopping when the patient has no other resource.
• Take vitals when assigned by the RN.
• Provide compassionate emotional support to patients and families.
• Demonstrate active listening and offer encouragement.
• Maintain patience, kindness, and professionalism.
• Report changes in condition to the RN immediately.
• Follow individualized care plans and policies.
• Practice Standard Precautions and infection-control requirements.
• Maintain required competencies and skills.
• Participate in RN supervision and evaluations, including 14-day check-ins.
• Participate in annual performance evaluations.
• Complete at least 12 hours of in-service education each year.
• Maintain current certifications and credentials.
• Knowledge of infectious disease protocols and infection-control practices.
• Work collaboratively with the Hospice team for quality end-of-life care.
