Job summary

Boston Medical Center seeks a Hospice Home Care Nurse to assess patient and family needs, plan and deliver terminally ill care in home settings, and coordinate with the interdisciplinary team. Requirements include a Diploma/ADN/BSN, current Maryland RN license, and strong communication and time‑management skills.

Qualifications

• Diploma/ADN/BSN from an accredited program.
• RN licensure valid in Maryland or applicable state.
• Experience in home care or hospice preferred.

Responsibilities

• Completes initial assessment of patient and family needs for home care.
• Prepares and implements the care plan with the Interdisciplinary Group.
• Provides direct nursing care and coordinates with the HHA.
• Updates physicians and interdisciplinary team; participates in rounds.
• Educates family and facility staff on care tasks and meds.
