Hopestone Hospice is seeking a Field RN (Registered Nurse) in Fort Worth, TX for a full-time role. The RN will assess patients at home or in care settings, develop and document care plans, and coordinate with the interdisciplinary team to address physical, emotional, and spiritual needs.

The role emphasizes patient comfort, family support, and quality end-of-life care with on-call responsibilities and ongoing education for staff. Requires an active RN license and CPR certification.

Formación

• Current RN License required.
• CPR Certification required.
• Valid Driver's License required.
• Valid Auto Insurance required.
• Hospice RN experience of 2+ years preferred.
• HomeCare HomeBase experience preferred.

Responsabilidades

• Initial and ongoing assessments of patients' physical, functional, psychosocial, and environmental needs.
• Develop and implement the plan of care to ensure quality and continuity of care.
• Educate patients and families about disease process, end-of-life care, and self-care techniques.
• Provide training to other staff as needed.
• Determine scope and frequency of services based on acuity and family needs.
• Provide on-call services to patients and families as assigned.
