Hopestone Hospice is seeking a PRN Hospice Field RN to provide one-on-one care to patients in home settings throughout Weatherford, TX. You’ll exercise clinical judgment in the field with support from an interdisciplinary team, delivering dignified end-of-life care and education to families.

The role emphasizes autonomy, flexibility, and compassionate patient-centered service, with opportunities to influence care plans while maintaining work-life balance through a flexible PRN schedule.

Qualifications

• Active RN license in good standing.
• Current CPR certification.
• Valid driver’s license and auto insurance.
• Minimum 1 year hospice experience preferred (2+ strongly preferred).
• Experience with Homecare Homebase is a plus.
• 401(k) eligibility after 3 months with company match details.

Responsabilités

• Perform initial and ongoing comprehensive assessments of patients' needs.
• Determine hospice eligibility, admission, and recertification using criteria.
• Develop, implement, and adjust individualized plans of care.
• Provide education and emotional support to patients and families.
• Coordinate with the interdisciplinary group (IDG).
• Assess caregiver capability and identify support needs.
• Supervise and provide direction to Hospice Aides and LVN/LPNs.
• Document timely and accurate clinical notes.
• Communicate changes in patient condition and adjust care plans.
