Job summary

Interim HealthCare of the Upstate is seeking a compassionate Hospice Aide (CNA) to provide patient-centered end-of-life care in home settings across South Carolina. You will deliver clinical excellence with empathy, supporting patients and their families during a meaningful transition.

Qualifications

• High school diploma and SC CNA certification.
• Valid driver’s license, auto insurance, and reliable transportation.
• Compassionate nature with strong communication skills; ability to lift up to 50 lbs.
• Hospice experience preferred.

Responsibilities

• Care for seniors in patients' homes.
• Assist with activities of daily living such as bathing, toileting, dressing, grooming, hygiene, and nutrition.
• Assist with ambulation, transfers and ROM exercises.
• Check vitals, monitor intake/output, and document changes.
• Work with home health nurses to follow the patient’s care plan.
• Observe patients and notify supervisor of issues requiring changes in care or equipment.
