Job summary

Hospice Savannah is seeking a PACE Certified Nursing Assistant (CNA) to join the IDT in Savannah, GA. You will provide personal care per individual care plans for PACE participants, maintain confidentiality, and advocate for patient rights while following agency, state, and federal directives.

You will identify cultural and lifestyle differences, interact with diverse populations, and collaborate with physicians, staff, and families to ensure compassionate, high-quality care.

Qualifications

• Current Georgia Nurse’s Aide certification is required.
• CPR certification is required.
• Two+ years of direct patient care experience in hospice, hospital, skilled nursing, assisted living, or home health.
• Valid driver's license with reliable transportation.

Responsibilities

• Provide personal care in accordance with individual care plans for PACE participants.
• Maintain patient confidentiality and promote patient rights as supported by directives.
• Demonstrate empathy, effective communication, and teamwork with the IDT and families.
