Job summary

St. Croix Hospice in Baraboo, WI is seeking a dedicated Hospice Aide (CNA) to join our interdisciplinary care team.

You will provide compassionate, hands-on support to patients in their homes under the supervision of a Registered Nurse, assisting with personal care and daily activities to promote comfort and quality of life. The role requires current CNA certification, a valid driver’s license, and willingness to travel to patient homes.

Qualifications

• Current CNA certification in the state of practice.
• Record vital signs and report observations to the care team.
• Provide personal care including bathing, skin/nail care, oral hygiene, dressing/undressing.
• Travel to patient homes throughout the day and provide care as scheduled.

Responsibilities

• Provide compassionate, hands-on support to patients in their homes under RN supervision.
• Assist with daily living activities to promote comfort and dignity.
• Maintain accurate documentation of services provided.
