Job summary

Bluegrass Care Navigators Hospice Team seeks a dedicated Certified Nurse Assistant to join our home-based care team, delivering compassionate personal care and support to patients and families within their homes and care settings.

The CNA works under RN supervision to observe and document patient needs, ensuring comfort and safety, with a schedule aligned to Monday–Friday operations. Must be a licensed driver and have a valid KY CNA registration.

Qualifications

• Must be at least 18 years old and a high school graduate or equivalent.
• Currently registered as a State Nurse Aide with the Kentucky Board of Nursing.
• Completion of a recognized Certified Nurse Assistant (CNA) training program.

Responsibilities

• Provide personal care and support to meet daily needs and maintain a safe environment.
• Observe patient condition and accurately report and document observations and care provided.
• Work under RN supervision to ensure quality hospice care.
• Maintain clear, timely documentation of services delivered for patient records.
