Job summary

Gentiva Hospice is seeking a dedicated Hospice Aide/CNA to provide compassionate personal care and emotional support to patients at home or in hospice facilities. You will assist with daily living activities, monitor patient condition, and document visits as part of a coordinated care team.

The role requires an active CNA license, CPR certification, a valid driver's license and reliable vehicle, and at least 6 months of direct patient care experience.

Qualifications

• Active CNA license in the state of employment (Required).
• Current CPR certification (Required).
• Driver’s license and reliable vehicle with insurance (Required).
• Six (6) months of direct patient care experience (Required).

Responsibilities

• Provide direct personal care to terminally ill patients.
• Assist with personal hygiene, grooming, and toileting.
• Support safe mobility using walkers or wheelchairs.
• Help with meal preparation, feeding, and light housekeeping.
• Observe and report changes in patient condition.
• Offer emotional support to patients and families.
• Maintain accurate documentation of visits.
• Attend team meetings and participate in QI activities.
