Job summary

Adoration Health, an affiliate of BrightSpring Health Services, is seeking a Hospice CNA in Altoona, PA to provide compassionate 1:1 patient care, with scheduling flexibility and strong team support. You will assist with daily activities, monitor patient condition, and help families during a difficult time.

Qualifications

• Meet the training requirements of the state of practice.
• Minimum six months experience in providing personal care.
• Must be 18+ years of age.
• Complete 75 hours of classroom and supervised training, 16 of which are prerequisites.
• Possess and maintain current CPR certification.
• Read and follow written instructions and document care.
• Understand hospice philosophy and provide specialized end-of-life care.
• Work with little direct supervision.
• Able to handle death/dying and provide emotional support.
• Strong communication and organizational skills.

Responsibilities

• Personal Care: bathing, dressing, toileting, mobility and repositioning.
• Monitor & Report: observe and report changes in patient condition.
• Mobility: assist with ambulation, transfers, ROM and safe equipment use.
• Medication Assistance: help with self-administered meds per state rules.
• Emotional Support: provide companionship to patients and families.
• Light Housekeeping: change linens, assist with meals and keep space clean.
• Documentation: maintain accurate and timely patient records.
• Team Participation: collaborate with hospice interdisciplinary team.
