Qualifications

• Medical Assistant certification preferred (CMA/RMA or equivalent).
• Experience in hospice, palliative care, home health, geriatrics, primary care, or care coordination required.
• Strong communication, customer service, and interpersonal skills.

Responsibilities

• Coordinate patient appointments, home visits, and follow-up care.
• Communicate with patients, families, providers, referral sources, pharmacies, and community partners.
• Document patient updates, symptoms, and clinical information in the EMR.
• Process provider orders, medication changes, and care plan updates.
• Assist with hospice and palliative care referrals, admissions, and scheduling.
• Support care transitions between hospitals, home health, palliative care, and hospice services.
• Manage inbound and outbound calls, messages, and patient inquiries.
• Provide compassionate support and education to patients and families.
• Identify and communicate changes in patient condition to the clinical team.
• Assist with advance care planning documentation and care coordination.
• Maintain accurate and timely records while ensuring compliance with agency procedures
