Job summary

Geisinger Hospice-Selinsgrove, part of LHC Group and under UnitedHealth Group's Optum family, seeks a Patient Care Manager - Hospice to support Executive leadership in delivering safe, efficient patient care.

You will ensure regulatory compliance, promote quality and safety, resolve issues, and mentor staff. A current RN license and reliable transportation are required; CPR preferred. Salary ranges from $82,004 to $123,006 annually.

Qualifications

• Current unrestricted RN licensure in state of practice.
• Current driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation.
• Preferred CPR certification or ability to complete within 90 days of hire.
• 4+ years full-time experience as an RN or equivalent.
• 1+ years of clinical experience.
• Able to work independently.
• Good communication, writing, and organizational skills.

Responsibilities

• Responsible for compliance with hospice policies and procedures and with local, state, and federal regulatory requirements governing the practice of the assigned team.
• Actively promotes and directs assigned team regarding quality of care and safety of patients and staff.
• Engages in thorough problem resolution and complaint investigation.
• Provides in-services to staff to promote the health and safety of the patient, to familiarize staff with regulatory issues and agency policy and procedures, and to address any identified educational needs.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
