Job summary

Ohio's Hospice of Dayton is seeking a Clinical Coordinator to provide administrative support to clinical operations and team leaders in Wooster, OH. The role includes phone support, coordinating care, and ensuring access to resources for patient care.

The candidate will work 5 days a week, with occasional travel to the New Philadelphia office and other Ohio's Hospice locations as needed. This role emphasizes compassionate service and efficient operations within a hospice setting.

Qualifications

• Associate degree in office management or related field preferred.
• Proficiency in Microsoft Word and Microsoft Excel.
• Previous EMR system experience preferred.
• Excellent communication and interpersonal skills.
• Strong organizational skills and attention to detail.
• Ability to prioritize multiple responsibilities in a fast-paced environment.
• Valid driver’s license with a safe driving record.
• State minimum automobile insurance coverage.

Responsibilities

• Prepare and distribute daily care manager caseload reports to team leaders.
• Distribute lab work to care managers to ensure timely access to patient information.
• Maintain PTO, holiday, on-call, and attendance records for assigned teams.
• Coordinate meetings including booking space and recording minutes.
• Handle patient and family calls and communicate with the team.
• Document patient and family calls in the EMR promptly and accurately.
• Prepare and distribute Quality of Life materials for meetings.
• Review payroll for accuracy and complete payroll responsibilities.
• Complete audits and reports as requested by leadership.
• Travel to other Ohio's Hospice locations as needed.
