Qualifications

• Required to assist patients with personal care and ensure their comfort.
• Must perform observations and report accurately to the Case Manager.
• Experience in a long-term care or dementia facility preferred.

Responsibilities

• Perform personal care tasks as assigned.
• Assist with patient mobility and daily activities.
• Report significant changes in patients’ conditions.

Proficient in using an iPhone for electronic documentation

Education

High school Diploma or GED
