Job summary

Hillcrest Home Care Hospice is seeking a compassionate Hospice Assistant to join our team. You will provide hands-on personal care to patients in home and residential settings, following an RN-developed plan of care and supporting families with dignity and comfort.

The role requires CNA certification, a valid driver’s license, and reliable transportation, with experience in home care or hospice preferred. Travel to multiple patient locations is expected, with ongoing collaboration alongside the

Qualifications

• Completion of Nursing Assistant coursework required.
• High school diploma or equivalent preferred.
• At least two years of experience providing personal care.
• At least one year of Home Care or Hospice experience preferred.
• Active CNA certification.
• Valid driver’s license required.
• Reliable transportation required.
• Current automobile insurance required.

Responsibilities

• Provide personal care according to the individualized hospice plan of care developed by the RN.
• Assist patients with activities of daily living (bathing, grooming, dressing, toileting).
• Provide nutritional support as outlined in the plan of care.
• Document vital signs, weights, and other data.
• Assist with light housekeeping to maintain a safe environment.
• Assist with medications as permitted within care plan.
• Observe changes in patient condition and report promptly to RN.
• Complete accurate documentation of all care provided.
• Provide compassionate support to patients and families.
• Communicate with patients, families, nurses, and care-team members.
• Document time spent on visits, travel, and meetings.
• Order, pick up, and deliver patient supplies as needed.
• Follow hospice procedures and care plans for continuity of care.
• Participate in on-call and backup call responsibilities.
