Luminary Hospice is seeking a compassionate hospice aide to support patients in their end-of-life journey under RN supervision. You will perform personal care, observe patients, document care provided, and help maintain a safe and caring home environment.

Responsibilities include assisting with bathing, mobility, meals, medication reminders where allowed, and collaborating with the Case Manager to monitor patient status.

Formación

• Active STNA or CNA certification required in the practice state.
• Minimum 75 hours of classroom and supervised practical training; 16 hours of classroom before 16 hours of practical.
• Must be at least 18 years old.
• Ability to read and follow written instructions and document care.
• CPR certification current.
• Licensed driver with insured vehicle and clean record.

Responsabilidades

• Perform personal care tasks after patient assignment and report if unable to perform a task.
• Provide patient and family with compassionate communication and confidentiality.
• Assist with safety needs and proper use of equipment.
• Assist with bathing, grooming, feeding, and daily activities as assigned.
• Record vital signs and report significant changes to Case Manager.
• Assist with ambulation, ROM, and transfers as directed.
• Maintain clean, orderly living area and follow care procedures.
