Job summary

Concho Hearts Hospice is seeking a compassionate PRN Hospice CNA/Aide to support patients in home settings across Wise and Montague Counties. This flexible schedule lets you care for patients on your terms while delivering comfort and dignity.

What you'll do: provide personal care, assist with daily living activities, monitor patient conditions, document care, and coordinate with the interdisciplinary hospice team to keep families informed and supported.

Qualifications

• Active CNA certification in Texas.
• Compassionate, patient-centered approach to care.
• Strong communication and teamwork skills.
• Reliable transportation and willingness to travel locally.
• A passion for providing comfort and dignity to hospice patients.

Responsibilities

• Provide compassionate personal care to hospice patients in their homes or care facilities.
• Assist with daily living activities while ensuring patient comfort and dignity.
• Observe and report patient condition changes to the nursing team.
• Document care and communicate with the interdisciplinary hospice team.
• Build meaningful relationships with patients and their families.
