Job summary

Choice Hospice, Inc. is seeking a Hospice Aide (HHA) PRN in Fort Worth, TX. The role involves direct patient care under the supervision of an RN, assisting with transfers, reporting changes, and maintaining a clean, safe home environment.

Aide will support patients and families while following the plan of care and agency policies. The ideal candidate holds CNA/HHA and LTC certifications, a valid driver’s license, CPR, and preferably HomeCare HomeBase experience.

Qualifications

• Current CNA/HHA & LTC certifications.
• Valid Driver's License.
• Valid Auto Insurance.
• CPR Certification.
• HomeCare HomeBase experience preferred.

Responsibilities

• Provide direct patient care to patients under direction of the RN and according to the Aid Plan of Care.
• Assist in safe transfers and ambulation per agency policy.
• Report changes and document pertinent information for continuity of care.
• Practice infection control; maintain a clean, safe environment.
• Promote respectful, clear communication with patient/family and staff.
