Job summary

The Choice Hospice is seeking a Hospice Aide (HHA) for PRN assignment in Fort Worth, TX. You will provide direct patient care under RN supervision, support transfers, and assist with daily activities while maintaining a safe, compassionate environment.

CPR and state CNA/HHA/LTC certifications are required. Applicants should have a valid driver’s license, auto insurance, and preferably HomeCare HomeBase experience.

Qualifications

• Current CNA/HHA & LTC certifications.
• Valid Driver's License.
• Valid Auto Insurance.
• CPR Certification.
• HomeCare HomeBase experience preferred.

Responsibilities

• Provide direct patient care under RN direction and the Aid Plan of Care.
• Assist with safe transfers and ambulation per policy.
• Report changes and document care rendered to ensure continuity.
• Maintain infection control; keep environment clean and safe.
• Promote respectful, clear communication with patient/family and staff.
• Respect patient privacy and property.
• Assist with proper nutrition and fluid intake.
• Support efficient operation and time management for the agency.
• Perform treatments and procedures per policy.
• Support agency philosophy and policies to ensure quality of care.
