Job summary

Choice Health at Home seeks a Hospice Aide (HHA) PRN in Fort Worth to provide direct patient care under the RN supervision. You will assist with personal care, safe transfers, and maintain a clean, comfortable home environment in patients’ residences.

You must hold current CNA/HHA and LTC certifications, a valid driver’s license, auto insurance, and CPR. HomeCare HomeBase experience is preferred; strong communication with patients and families is essential to support the care team.

Qualifications

• Current CNA/HHA & LTC certifications.
• Valid Driver’s License.
• Valid Auto Insurance.
• CPR Certification.
• HomeCare HomeBase experience preferred.

Responsibilities

• Provide direct patient care to patients under direction of the RN and according to the Aid Plan of Care.
• Assist with safe transfers and ambulation per agency policy.
• Report changes and document care rendered to ensure continuity of care.
• Maintain infection control and provide a clean, safe environment.
• Communicate respectfully with patients, families and staff.
• Support nutrition and fluid intake and help with activities of daily living.
• Assist with treatments and procedures per agency policy.
