Job summary

Choice Health At Home is seeking a Hospice Aide (PRN) to provide direct patient care under RN supervision, promoting safety, comfort, and dignity in the patient’s home. You will assist with transfers, reporting changes, and maintaining infection control while supporting nutrition, privacy, and compassionate communication.

Responsibilities include documenting care, assisting with nursing plans, and contributing to efficient operations in a hospice setting.

Qualifications

• Infection control knowledge and ability to maintain a clean, safe environment.
• Ability to report changes and document care rendered for continuity of care.
• Excellent communication with patient, family and staff.
• Time management to support efficient care delivery.

Responsibilities

• Provide direct patient care under RN direction and Plan of Care.
• Assist with safe transfers and ambulation per policy.
• Record observations and care provided for continuity of care.
• Maintain infection control; keep environment clean and safe.
• Promote respectful communication with patient/family and staff.
• Support patient privacy and protection of property.
• Assist with nutrition and fluid intake as appropriate.
• Contribute to efficient operation of the agency and time management.
• Support agency philosophy and policies to ensure quality care.
