Job summary

TheraCare Home Health in Fort Worth, TX is seeking a qualified Home Health Aide (PRN) to assist patients in their homes. The role supports RN supervision and focuses on compassionate, patient-centered care.

Responsibilities include assisting with daily living activities, safe transfers, and reporting changes to the clinical team. Candidates must have completed a Certified Home Health Aide Program and be English-speaking with CPR certification.

Qualifications

• Completion of Certified Home Health Aide Program.
• 1 year of Home Health Aide experience in a home health agency.
• Reliable transportation with current auto liability insurance.
• Good communication with patients, caregivers and clinical teams.
• Ability to read, write and speak English.
• Demonstrates interest in the welfare of ill and elderly.
• Proof of CPR and Hepatitis profile.
• Aide Competency Evaluation Skills Checklist and Written Skills Test.

Responsibilities

• Provide direct patient care to patient under direction of the RN and according to the Aide Plan of Care.
• Assist with safe transfers and ambulation per agency policy.
• Report changes and document care rendered to ensure continuity of care.
• Practice infection control principles; maintain a clean, safe environment.
• Promote respectful communication with patient/family and staff.
• Respect patient privacy and property.
• Assist with nutrition and adequate fluid intake.
• Record clinical documentation in the medical record and participate in case conferences.
• Participate in interdisciplinary communication to support patient needs and progress.
