Job summary

Amedisysis seeking a hospice aide to support patients at home, helping with personal care and daily activities. You will follow the plan of care, assist with meals, mobility, and homemaking, and report changes to the nurse to ensure comfort and safety.

Qualifications

• High school diploma or equivalent required.
• Current CNA certification or active state registration required.
• CPR certification required.
• Valid driver's license and reliable transportation required.
• CNA experience within the last two years preferred.

Responsibilities

• Provides personal care services to patients.
• Assists with ambulation, transfers, ROM, and positioning.
• Plans and prepares nutritious meals and assists with feeding.
• Performs homemaking and environmental tasks as assigned.
• Observes and reports changes to the patient's status to the nurse.
• Follows written care instructions and documents care delivery.
• Assists patients with prescribed exercises as directed.
• Performs simple urine tests for sugar, acetone, and albumin.
• Other duties as assigned.
