Job summary

Beth Israel Lahey Health seeks a dedicated Hospice Aide to provide compassionate patient care across Woburn, Tewksbury, Malden, Melrose and nearby communities. The role is 40 hours weekly, Monday through Friday, 8:00 AM to 4:30 PM, with a focus on maintaining a safe, healthy home environment and supporting the IDT plan of care.

You will assist with personal care, hygiene, mobility, meals, medication prompting, and vital sign monitoring while documenting changes and communicating with the hospice

Qualifications

• Valid MA CNA or HHA certification required.

Responsibilities

• Provide personal care and homemaker services per plan of care.
• Assist with hygiene, bathing, grooming and related tasks.
• Assist with toilet needs and safe transfers using gait belts or equipment.
• Aid with meals and feeding as needed.
• Observe and document patient status; report to hospice nurse case manager.
