Job summary

Holisticare Hospice is seeking a Full Time Hospice Aide (CNA/HHA) to join our compassionate team, providing hands-on care that brings comfort and dignity to patients at the end of life. Working closely with our nursing team, you’ll ensure each patient’s physical, emotional, and personal needs are met with respect, empathy, and professionalism.

Schedule: Monday - Friday, 7 am - 3:30 pm. Coverage area: Monmouth & Middlesex counties.

Qualifications

• High School Diploma or G.E.D. required.
• Valid Certified Home Health Aide (HHA) license OR a valid Certified Nursing Assistant (CNA) license required.
• Minimum of 6 months experience in hospice or skilled nursing facility preferred.
• Current driver’s license, vehicle insurance, and access to dependable transportation.

Responsibilities

• Provide Personalized Care: Follow the carefully crafted plan of care to ensure the comfort and well‑being of our patients. Attend promptly to the requests of patients and their family/caregiver, offering compassionate and unwavering support.
• Assist with Personal Care: Support patients with various personal care needs, including bathing, oral hygiene, changing bedding, clothing, and shampooing.
• Be Dedicated to Customer Service: Emphasize exceptional customer service, going above and beyond to ensure optimal patient satisfaction.
• Provide Support for Families: Guide and support families caring for terminally ill loved ones.
• Maintain Accurate Documentation: Ensure accurate documentation in the home and agency records.
