Job summary

Heart to Heart Hospice is seeking a dedicated Hospice Aide (CNA) to join our care team in Michigan. You will provide direct patient care under the supervision of the RN and follow the Plan of Care to support comfort and wellbeing.

Applicants must hold an unencumbered CNA license (MI) or equivalent, have at least one year of related experience, and possess reliable transportation with a valid driver's license; EMR experience is a plus. We offer comprehensive benefits and opportunities for growth.

Qualifications

• High School Diploma or GED required or preferred.
• Unencumbered CNA certification required.
• Current CNA license.
• At least 1 year experience as Hospice Aide or Nursing Assistant.
• Demonstrates interest in welfare of ill and elderly; compassionate care.
• High integrity, including confidentiality of information.
• EMR experience a plus.
• Must have valid driver's license and reliable transportation.

Responsibilities

• Provide direct patient care to patient under direction of the RN and according to the Hospice Aide/Homemaker Plan of Care.
• Provide necessary skills to assist the patient with safe transfers and ambulation per agency policy.
• Provide necessary skills to report/communicate changes and document pertinent information and care provided to patient to ensure continuity of care.
• Practice accepted infection control principles.
• Promote positive, supportive, respectful communication to patient/family and other employees.
• Timely submission of all documentation.
• Perform other duties as required.
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures.
