Job summary

Heart to Heart Hospice in Kalamazoo, MI seeks a compassionate Hospice Aide (CNA) to provide direct patient care under the supervision of an RN and in accordance with the Plan of Care. You will assist with transfers, ambulation, reporting changes, and maintain infection control; you will document care and communicate with patient families; a driver's license and reliable transportation are required.

Qualifications

• High School Diploma preferred or GED.
• Must have an unencumbered CNA certificate.
• Current CNA license is required.
• At least one year experience as Hospice Aide or Nursing Assistant in healthcare settings.
• Demonstrates interest in welfare of ill and elderly; maintains confidentiality.
• EMR experience is a plus.
• Must have a valid driver's license and reliable transportation.

Responsibilities

• Provide direct patient care under RN supervision and Plan of Care.
• Assist with safe transfers and ambulation per policy.
• Report and document changes and care provided for continuity of care.
• Follow infection control principles.
• Promote respectful communication with patient and family, and other staff.
• Submit all documentation in a timely manner.
• Perform other duties as required.
• Ensure compliance with laws, Medicare regulations, and policies.
