Qualifications

Responsibilities

• Provide hands-on personal care and assistance in patient homes.
• Assist with ambulation Exercises and activities of daily living.
• Assist in medication administration limited to patient-initiated tasks (water/fluids).
• Collaborate with RN to develop and revise the plan of care.
• Document patient observations and communicate changes to the team.
