Qualifications

• Completion of state-required Hospice Aide/CNA training or CNA certification if required by state.
• Current CPR certification.
• Ability to follow care plans, document clearly, and communicate effectively.

Responsibilities

• Assist patients with bathing, dressing, grooming, toileting, and feeding.
• Support comfort through repositioning, mobility, and skin care.
• Provide light housekeeping to maintain a clean, safe home environment.
• Offer respite support to caregivers and families.
• Observe and report changes to RN/Case Manager.
• Assist with vital signs and basic care tasks as directed.
