Job summary

Qualifications

• Active CA CHHA license required.
• Minimum one year of patient care experience; preferable in Home Health.
• Licensed driver with insured automobile.

Responsibilities

• Providing personal care including baths, hygiene, and mobility support.
• Planning and preparing nutritious meals; assisting in feeding if necessary.
• Taking and recording vital signs as ordered; monitoring patient condition.
• Assisting patient in ambulation and exercise per care plan.
• Administering medications as directed and reporting changes to the nurse.
• Ensuring safety and proper use of equipment; maintaining a clean living space.
• Reporting on patient condition and significant changes to the assigned nurse.
