Compassus is seeking a Hospice Admissions Coordinator to process patient referrals, coordinate admissions, and enter admission data after all hospice requirements are met.
The role includes communicating with physicians, securing admission orders, coordinating RN assessments, and scheduling informed consent visits while ensuring chart accuracy and proper notifications to the IDT.

High school diploma or GED required.
Bachelor's degree preferred.
LPN preferred.
Two (2) to three (3) years of related experience and/or training strongly preferred.
An equivalent combination of education and experience will be considered.
Experience communicating with physicians, nurses and other healthcare personnel highly preferred.
Experience in insurance and Medicare/Medicaid billing and reimbursement preferred.
The demands of this role necessate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 25 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus.
