Remarkable Home Health in McKinney, TX seeks a Speech Therapist (PRN) to assess and treat speech-language, cognitive and swallowing disorders in home-based patients. You will collaborate with families, physicians and the care team to meet functional goals.

The ideal candidate has a Master’s or Doctoral degree in Speech-Language Pathology, 2 years of relevant home health experience, current state licensure and CPR. Must have a valid driver’s license; occasional travel to patient homes.

Qualificações

• Master's or doctoral degree in speech-language pathology.
• 2 years of experience relevant to Home Care or Hospice; adult home health experience preferred.
• Appropriate state licensure, driver's license and proof of current auto liability insurance; no listing in OIG Excluded Provider Listing. CPR current.

Responsabilidades

• Performs comprehensive assessment of speech-language, cognitive and swallowing disorders and identifies strengths and deficits.
• Establishes measurable short- and long-term goals with time frames based on initial assessment and modifies goals as needed.
• Selects therapy methods and provides treatment of speech-language, cognitive and swallowing disorders to meet functional goals.
• Provides education to patients, families and staff regarding diagnosis, treatment, and Plan of Care.
• Maintains accurate documentation of therapy visits and coordinates care with the physician.
• Makes visits as scheduled and participates in on-call rotation as required.
