Trio Home Health in Denton, TX is seeking a compassionate Speech-Language Pathologist PRN to serve patients in Denton County. The role involves planning, implementing, and evaluating care plans to rehabilitate and maintain patient well-being across speech, language, swallowing, and cognitive disorders.

Requires a Master’s or Doctoral degree in SLP and a current state license; CPR certification is required. Travel to local Denton County areas will be needed and excellent communication with

Qualificações

• Master's or Doctoral degree in Speech-Language Pathology.
• Current State License in Speech-Language Pathology or meets requirements in 42 CFR §484.115(n).
• One year of experience in acute care, rehabilitation, or home health preferred.
• Therapy skills in line with standards; good interpersonal skills; CPR.
• Reliable transportation and valid auto liability insurance.
• Perform ongoing clinical assessments, including OASIS, as appropriate.
• Develop and evaluate plan of care with patient and caregivers.
• Prepare and complete clinical notes, orders, and documentation timely.
• Analyze patient needs and provide appropriate care as ordered by physician.
• Provide therapeutic treatments for speech, language, cognitive, and swallowing disorders.
• Apply infection control and universal precautions.
• Accept clinical assignments and provide supervision to home health aides as appropriate.
• Communicate with physicians and health care practitioners as necessary.
• Provide patient, caregiver, and family counseling and education.
• Utilize effective interpersonal relations to achieve outcomes.
• Meet mandatory continuing education requirements and participate in QAPI.

Responsabilidades

• Provide speech-language therapy for speech, language, swallowing, and cognitive disorders.
• Develop and evaluate care plans with patient, family, and caregivers.
• Complete clinical notes, orders, and documentation in a timely manner.
• Coordinate with physicians and health care team to ensure coordinated care.
• Apply infection control and universal precautions in all visits.
• Travel locally to serve patients in Denton County.
