SYNERGY HomeCare seeks an experienced Registered Nurse for in-home care. You will deliver compassionate, life-affirming care, train staff, and evaluate patient needs within home health. The role emphasizes documentation, patient education, and collaboration with families.

The job requires an active RN license, a related bachelor’s degree, and strong communication and organizational skills. A valid driver's license and reliable transportation are expected.

Competenze

• Bachelor's degree in a related field.
• Active RN license.
• Experience in CHHA, Acute Care, Medical-Surgical, and Critical Care settings; Nursing Home background; strong assessment and documentation skills.
• Patient-oriented with strong interpersonal skills.
• Excellent communication skills (written and verbal) and computer skills.
• Strong attention to detail.
• Highly organized.
• Transportation and valid driver's license.

Mansioni

• Assist with employee training as needed, utilizing your expertise as a registered nurse.
• Evaluating the healthcare needs of patients and families with consideration of physiological, psychological, social, and environmental factors
• Evaluating & Completing Home Safety Checklist
• Implementing and revising the nursing care plan whenever necessary by regularly assessing, observing, and evaluating the patient's condition.
• Teaching the patient, family, and caregivers whenever appropriate and providing instruction regarding medication, diet, safety, and treatment modalities following the plan of care
• Handling other duties as needed
