Job summary

CenterWell Home Health in Colorado Springs is seeking a registered nurse to provide one-on-one home care, administer medications, perform wound care and develop care plans with physicians. You will work with a multidisciplinary team to support recovery and independence in patients' homes.

This role requires an unrestricted RN license, CPR certification, and a valid driver's license. You will educate families, document care, and supervise Home Health Aides while delivering compassionate,

Qualifications

RN licensure is required and must be current/unrestricted.
Experience in med-surg/ICU/ER settings.
CPR certification is required.
Must have valid driver's license and reliable transportation.

Responsibilities

Provide skilled nursing care to patients in their homes, including medications, wound care, and assessments.
Develop and implement individualized care plans with physicians and other professionals.
Monitor patient conditions and report changes.
Educate patients and families on disease management, medications, and treatments.
Maintain accurate records and coordinate with other healthcare professionals.
Report patient progress to physician and Clinical Manager.
Oversee Home Health Aides and ensure proper documentation.
