Job summary

CenterWell Home Health seeks a Home Health Registered Nurse to provide personalized, in-home nursing care, develop and implement care plans with physicians, and monitor patient progress. Qualifications

• Med surg, ICU, ER, or acute experience.
• Current and unrestricted Registered Nurse licensure.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Valid driver's license, auto insurance and reliable transportation.
• One year nursing experience (preferred).
• Home Health experience (preferred).

Responsibilities

• Provide skilled nursing care to patients in their homes, including administering medications and wound care.
• Develop and implement individualized care plans with physicians and other healthcare professionals.
• Monitor patients' conditions and report changes.
• Educate patients and families on disease management, medications, and treatment options.
• Maintain accurate records of patient care and coordinate with other healthcare professionals.
• Report patient care progress to physician and Clinical Manager.
• Oversee the work of Home Health Aides and monitor documentation.
