Job summary

CenterWell Home Health is seeking a compassionate Registered Nurse to provide skilled, one-on-one care in patients' homes. Reporting to the Clinical Manager, you will assess patients, administer medications, perform wound care, and develop individualized care plans with physicians.

You will educate patients and families, coordinate with a multidisciplinary team, and ensure accurate documentation. The role requires valid RN licensure, CPR, a valid driver's license, and reliable transportation.

Qualifications

• Med surg, ICU, ER, or acute care experience required.
• Current and unrestricted RN licensure.
• Current CPR certification.
• Experience collaborating with a medical team.
• Valid driver's license with reliable transportation.

Responsibilities

• Provide skilled nursing care to patients in their homes, including meds, wound care, and assessments.
• Develop and implement individualized care plans with physicians and other clinicians.
• Monitor patient conditions and report changes promptly.
• Educate patients and families on disease management and treatment options.
• Maintain accurate patient care records and coordinate with the care team.
• Oversee Home Health Aides and ensure proper documentation.
