CONSUMER DIRECTED SERVICES INC. is seeking a Registered Nurse for Home Care Hospice & Palliative care. You will assess patients, develop individualized care plans, administer medications, and provide wound and pain management in patients' homes.

Collaborating with an interdisciplinary team, you will educate families, maintain documentation, and participate in on-call rotations to ensure compassionate, patient-centered end-of-life care.

Qualifications

• Current Registered Nurse licensure.
• Strong assessment, clinical judgment, and symptom management skills.
• Ability to deliver empathetic, dignity-focused end-of-life care.
• Medication administration, wound and pain management competence.
• Clear communication and ability to educate families.
• Documentation accuracy and teamwork with care teams.

Responsabilités

• Assess patients and create/adjust individualized care plans.
