CONSUMER DIRECTED SERVICES INC. is seeking a compassionate RN Case Manager to provide end-of-life hospice care in patients' homes and community settings. The role emphasizes What Matters, Medications, Mentation, and Mobility to guide individualized care.

You will assess, plan, implement, and evaluate care, supervise hospice aide staff, coordinate services, and participate in interdisciplinary team meetings, delivering patient-centered care with empathy and integrity.

Formación

• Current unrestricted RN licensure in Ohio.
• Valid driver's license and auto insurance.
• Minimum of two years RN experience, preferably in hospice or palliative care.

Responsabilidades

• Manage patient/family caseload, including assessments and care planning.
• Supervise and instruct hospice aide staff.
• Coordinate medication and supply delivery outside regular hours.
• Participate in interdisciplinary team meetings during on-call periods.
• Admit eligible patients for hospice care under physician orders.
• Report patient status and coordinate with healthcare teams.
• Ensure documentation and care notes are complete and accurate.
• Deliver patient-centered care aligned with core values of compassion and integrity.
