Diamond Care Group is seeking Home Health Registered Nurses in Orlando, FL for full-time and PRN positions. The role involves comprehensive patient assessments, care planning, and coordination with a multidisciplinary team to promote patient health at home.

Candidates must be licensed in Florida with at least one year of home health experience and be capable of traveling between patient homes. You will educate patients and caregivers, monitor medications and vitals, and collaborate with

Qualificações

• Current Florida RN license.
• At least 1 year of home health nursing experience.
• Knowledge of nursing care methods and safety guidelines.
• Strong communication and interpersonal skills; team player.
• Ability to work independently and manage a daily schedule; travel to patient homes.
• Familiarity with Kinnser EHR.

Responsabilidades

• Perform comprehensive patient assessments and develop individualized care plans with the care team.
• Complete evaluation tasks, including reviewing medications and vital signs.
• Coordinate care by making referrals or documenting clinical rationale when referrals are not indicated.
• Communicate with the clinical manager and interdisciplinary team regarding patient status and care updates.
• Educate patients and caregivers to promote independence and improve health outcomes.
• Coordinate with physicians, PTs, OT, and other care plan participants.
• Supervise and support LPNs to maintain high care standards.
• Perform additional duties to support patients and team operations.
