Qualifications

• Graduate from an accredited School of Nursing.
• Active Florida RN license required.
• One or more years of community/home health experience preferred.
• Knowledge of Medicare and Medicaid guidelines.
• Working knowledge of home healthcare and required documentation.
• Strong organization and time management.
• Ability to communicate constructively with nursing managers and staff.
• Criminal background check required.
• Current CPR certification; online certification not accepted.

Responsibilities

• Provide skilled nursing and comprehensive assessments to establish individualized care plans.
• Develop and maintain a therapeutic relationship by educating patients and caregivers on disease processes, medications, treatment plans and plans of care, promoting independence.
• Utilize EMR to document, process and maintain all documentation in compliance with agency standards.
• Facilitate communication between the patient and the interdisciplinary team.
