Job summary

UnitedHealth Group is seeking a Full Time Registered Nurse (RN) for Home Health in Monroe, Louisiana. The RN will provide and direct nursing care in patients' homes, coordinating plans of care with physicians and the health care team to ensure continuity of care and compliance with policies.

Responsibilities include initiating the plan of care with the physician, supervising home health staff, conducting comprehensive assessments, and documenting visits per policy.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• 1+ years of clinical RN experience.
• Current CPR Certification or ability to complete within 90 days of hire.

Responsibilities

• Initiates and revises the plan of care with physician and team.
• Supervises home health aides and licensed nurses per regulations; provides instruction and assigns tasks.
• Performs comprehensive assessments (OASIS) and medication reconciliation accurately and timely.
• Communicates with patients, families, and clinicians to ensure quality care and service excellence.
