UnitedHealth Group is seeking a Registered Nurse in Home Health to provide and direct nursing care to patients in their homes, in accordance with physician orders and agency policies. The RN will coordinate total plan of care with the health care team and ensure seamless patient care across settings.

Primary responsibilities include developing and updating the plan of care, supervising home health aides, and performing supervisory visits.

Formación

• Current and unrestricted RN licensure in state of practice.
• Ability to work independently in home health settings.
• Current CPR certification or ability to obtain within 90 days.
• 1+ years of Home Health experience preferred.
• Valid driver's license and access to reliable transportation.

Responsabilidades

• Initiates and revises the plan of care in collaboration with physicians and health care professionals.
• Supervises care by home health aides and licensed nurses; provides instruction and assigns tasks.
• Provides supervisory visits and ensures adherence to plan of care and regulatory standards.
• Completes comprehensive assessments and accurate documentation for payer requirements.
