Evergreen At Home in Oshkosh is actively seeking a compassionate Registered Nurse (RN) to deliver skilled home health nursing and help patients maintain independence in their own homes. You will collaborate with physicians, therapists, patients, and families to coordinate care and manage chronic conditions.

You’ll work 30-40 hours per week with a daytime schedule, a small driving radius within 45 minutes, and a supportive team environment.

Qualifications

• Active Wisconsin RN license.
• Current CPR certification.
• Valid Wisconsin driver's license.
• Reliable transportation with adequate auto insurance and good driving record.
• Strong organizational and time-management skills.
• Passion for serving older adults and helping them maintain independence.

Responsabilités

• Provide skilled nursing care to older adult patients in their homes.
• Coordinate care plans with physicians, therapists, and other healthcare providers.
• Monitor patient conditions and communicate changes promptly.
• Educate patients and families on disease management, medications, and care plans.
• Complete accurate OASIS documentation.
• Assist with discharge planning and patient transitions of care.
• Connect patients and families with community resources and support services as needed.
