Job summary

Symbii Home Health and Hospice-Layton is seeking a dedicated Home Health RN Case Manager to coordinate skilled nursing care in patients’ homes. You will assess patients, develop individualized plans, and coordinate interdisciplinary services to ensure quality care and regulatory compliance.

The role requires completing SOC/ROC/Recertifications, managing a caseload, and providing education to patients and families while maintaining EMR documentation and HIPAA standards.

Qualifications

• Current RN license in the applicable state.
• Graduate of an accredited school of nursing.
• Minimum of one year of nursing experience.
• Valid driver’s license and reliable transportation.
• Ability to travel throughout the assigned service area.
• Strong assessment, organizational, and communication skills.
• Basic computer and EMR proficiency.

Responsibilities

• Perform comprehensive patient assessments and develop individualized plans of care.
• Complete SOC, ROC, Recertification, Transfer, and Discharge assessments.
• Provide skilled nursing care in accordance with physician orders and agency policies.
• Monitor patient progress and update care plans as needed.
• Administer medications and treatments as ordered.
• Perform wound care, catheter care, infusion therapy, and other skilled nursing procedures.
• Identify changes in patient condition and communicate findings to physicians and appropriate care team members.
• Manage an assigned caseload of home health patients and coordinate care among providers.
• Ensure timely scheduling and completion of visits; optimize resource utilization.
• Participate in case conferences and interdisciplinary team meetings.
• Document accurately in EMR and ensure compliance with HIPAA and regulatory requirements.
• Educate patients and caregivers on disease processes, medications, safety, and self-management.
