Adoration Health in Westminster, MD seeks a dedicated Home Health Physical Therapist to provide 1:1 patient-centered care in a supportive, team-oriented environment.

The role requires a Bachelor's degree in Physical Therapy, an active state license, and CPR certification, with at least 2 years of PT experience preferred. Full-time schedule, flexible benefits, and growth opportunities are offered.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited program.
• New graduates may be considered in select markets based on program availability.
• At least 2 years of experience as a physical therapist; community/hospice experience preferred.
• Valid license from the state of practice.
• Possess and maintain current CPR certification.

Responsabilités

• Perform PT evaluation and treatment based on physician orders, including adaptive equipment.
• Set measurable goals and develop treatment plans to maximize independence.
• Maintain communication among treatment team, physician, patient, and family.
• Maintain thorough and timely documentation for proper billing.
• Supervise PTAs and PT aides in direct patient care as per state act.
