Job summary

EverHome Healthcare is seeking a part-time Physical Therapist to join our expanding home health team serving Snohomish, King, and Pierce counties. The role includes patient assessments, plan-of-care development, and delivering PT visits in clients' homes.

We value education of patients and caregivers on exercises and device use, with documentation in a simple charting system. Per-diem or full-time options may apply depending on needs.

Qualifications

• Licensed Physical Therapist in Washington state.
• BLS certification is required.
• Experience in home health is a plus.

Responsibilities

• Assess and evaluate therapeutic/functional status of clients.
• Develop clients' plan of care.
• Provide Physical Therapy visits in clients' homes.
• Instruct patients, families and caregivers in use and care of therapeutic appliances.
• Chart progress notes.
• Perform skills verification and annual performance for other Physical Therapists.
